Checklist

I have performed troubleshooting on my device and am 100% sure the device is faulty.

I acknowledge that the USB port is not covered under warranty and that the utmost care has been taken with the device.

I acknowledge that if e-liquid, foreign substances (eg. sand) or any evidence of missuse such as dropage is found, then this can void warranty if this is deemed to be the reason for the devices failure to function properly.

I have removed the batteries (where possible) and removed any liquid from the device before sending back (if the issue is with the mod only there is no reason to send back the tank).

I understand that if the device is returned and is found to be in perfect working order then I the customer will be responsible for the paying of postage to have it returned to me.

I have inluded my name, order number and a brief discription of the problem in the return parcel (feel free to print this form to inlude in the return parcel before pressing send).

I have filled in this form to the best of my knowledge and will send back the device which I believe to be faulty ASAP.
